Services We Provide

Physician Services

Our Medical Director provides medical guidance for the team, is available for consultation, and participates in patient/family care planning. The PhysioCare Hospice Medical Director will work with the patient’s Primary Care Physician (PCP) to ensure all medical needs are addressed.

Medical Social Services

Our Social Workers are available to provide coping skills to the patient and family during this difficult as they deal with new health issues and concerns. The Social Worker has connections with many community resources for medical, psychological, or social assistance. The Social Worker also acts as a confidential source to report concerns.

Skilled Nursing

Our compassionate nurses thoroughly assess each patient to help develop and implement the plan of care, as prescribed by the PCP. Our nurses provide specialized services including lab draws, bowel and bladder management, catheter care, attention to cardiac and lung care, wound care, infusion therapy and pain control, among others. Plus, our nurses will contact the primary care physician if additional medical conditions arise.

Counseling Services

In addition to social services, our Social Workers and Chaplain can arrange for short-term therapy and grief/bereavement counseling, and can suggest support groups, when appropriate. Also, our Chaplain can provide emotional and spiritual support for patients and loved ones. 

Home Health Aides

Our certified home heath care aides provide care under the supervision of a registered nurse or a licensed therapist. Typical duties include assisting with personal care services such as bathing, grooming, dressing, transferring, and assistance with feeding.

Counseling Services

Our dietitian evaluates the client’s dietary needs and provides advice and recommendations regarding appropriate nutritional intake as the patient’s condition progresses.

Volunteer Services

Volunteers and Homemakers provide many services to assist families, including running errands, grocery shopping, planning and preparing meals, assisting patients with self care, keeping the home clean and orderly,

transportation to medical appointments, and providing emotional support to the patient and family/caregivers.

Bereavement Services

Our bereavement cooridinator will give you and your family some next steps in the greaving process after your loved one passes away. This will consist of personal visits, phone calls and mailings. Times of when grief groups are available throughout the year through PhysioCare and other agencies will be shared with you as well.
